Currently the "Contact Us" page only allow us to put your contact info such as address and email etc,

I want to add a form which will allow visitors to put their requests or feedback directly to it and when they click the submit button, the message will be forwarded to my email address automatically.

This is a very commonly-used function in many websites. Can I add such a form to WA "Contact us" page? and How?

We do not have a built-in feature for this but you can add a generic HTML form or third-party widget like wufoo (See http://wufoo.com/ : http://wufoo.com/forums/viewtopic.php?id=804 )
 

A free solution with very little (or none) limitations is Zoho CRM or zoho DB.

Zoho CRM allow to collect Web-to-lead or Web-to-case forms. It similar to SalesForce just free (up to 3 users), See Zoho.com
